New Jersey physical connections are tested QUARTERLY on NJDEP form BWSE-CITR-IQ. Mail the completed form to the water supplier AND the local Administrative Authority (health board) within 5 days of the test; it goes to NJDEP only with permit applications/renewals (N.J.A.C. 7:10-10).
